To obtain Wellspring initial intake paperwork "click" button
After printing out these initial forms we request that you fill out all information and give them to your counselor on the day of the first session.
In addition to this initial paperwork we request you submit two additional pieces of information:
The first is your insurance card. We request this even if we are not participating providers and we are not billing for you.
Second, we ask that you hand in a credit card so we can make a copy of it. This card must be on file before treatment will start.
Finally, we ask that you arrive 15 minutes earlier than your scheduled appointment time for any additional paperwork you may need to fill out.
